TO REGISTER:
• Complete Registration Form • Select Sessions and Time Slots • Indicate T-Shirt Size • Submit $150.00 deposit per child & $12.00 T-shirt fee. (The deposit will be applied to your last scheduled session, providing all sessions signed up for were paid in full.) • Once you are confirmed for your session selection, you are obligated to pay the full session fee. The State of Connecticut requires that the health form be in compliance wit the Shelton School system's requirements. Therefore, a photocopy of your child's health form from his school file is acceptable.
If your child is entering kindergarten in September, we need a health form with the following information: Child's name, date of birth, up to date list of immunizations, a physical (must be within one year), doctor' signature and stamp.
The completed and correct form must be on file before your child can attend summer camp.
The Hide Out T-shirt
Children must wear their Hide Out T-Shirts on all trips including pool days. The T-Shirt this year will be orange & black tye dye with our logo in black. These T-Shirts will aid us in distinguishing our camp children when we are on trips. The counselors will have the same T-Shirts with the addition of “STAFF” on the back.
